Inspector Notes

An unannounced monitoring inspection was conducted from 3:30 - 4:15 pm with staff. There were nine children, ranging from five years to ten years of age, present with three staff supervising. Children were observed completing an arts and crafts activity and transitioning to homework time. Two onsite staff records and two onsite child records were reviewed at the time of the inspection along with the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary restrictions, the first aid kit, emergency supplies and the required posted information. Two staff records were reviewed off site virtually on 12/13/2022. Based on review of two staff records, it was determined that one staff had a disqualifying Office of Background Investigations (OBI) fingerprinting-based criminal history record check. Evidence: the record for staff #1 (date of hire: 10/24/2022) had an OBI determination letter that stated, "not eligible." Staff #1 was observed supervising children.
Plan of Correction: Staff #1 was terminated.
