Inspector Notes

An unannounced renewal inspection was conducted from 4:15 -5:00 pm with staff. There were seven children, ranging from five years to ten years of age, present with two staff supervising. Children were observed completing homework and reading. Three onsite staff records and two onsite child records were reviewed at the time of the inspection along with the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary restrictions, first aid kit and emergency supplies, the required posted information and five emergency medications, the medication log and their authorization forms. There were two staff on site with current Medication Administration Training (MAT) certification. Three staff records were reviewed off site at YMCA office on 4/14/2023.

Based on observation, and staff interviews, the center failed to have a second staff member or volunteer on the premises when one or more children are present. Evidence: at approximately 4:28pm, staff #2 left the building. Staff stated that they had to go to another job. At approximately 4:30 pm, staff #3 arrived at work. For approximately two minutes there was only one staff member supervising the seven children in care.
Plan of Correction: Staff were spoken too and retrained to never leave before additional staff arrive to take their place supervising children.
