During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on and completed on 05/20/2025. The on-site inspection began at 3:00 p.m. and ended at 4:00 p.m. The inspector reviewed compliance in the areas listed above. There were 33 children present and two staff. The inspector reviewed eight children?s records and two staff records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/05/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff.

On 5/20/2025 child A (age 6 years) and child B (age 8 years) were observed running out of the cafeteria to use the restroom without the supervision of staff. The Licensing Inspector observed the children run out of the door and notified staff A that the children had left the room. Staff A ran to the hallway to retrieve them.
Plan of Correction: A staff meeting was held and discussed the importance of sight and sound. Staff understood and agreed to changes needed.
Standard 8VAC20-780-350-A
The required ratio for children school age eligible up to 9 years is one staff per 18 children.

On 5/20/2025 staff A was observed standing in the hallway with two children to allow them to use the restroom, leaving staff B in the cafeteria supervising 31 children. Staff confirmed that it is their everyday practice to stand in the hallway while groups of children use the restroom leaving one staff in the cafeteria.
Plan of Correction: A staff meeting was held and went over ratio and the importance of ratio. Staff understood and agreed to changes needed.
Standard 8VAC20-780-500-A
Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

The children used hand sanitizer instead of using soap and water before eating afternoon snack.
Plan of Correction: A staff meeting was held and went over the routine of snack time. Staff understood and agreed to changes.
Standard 8VAC20-780-510-L
Medication, except for those prescriptions designated otherwise by written physician's orders, shall be kept in a locked place using a safe locking method that prevents access by children.

Four medications were observed stored in a box that was not locked. The medication box is stored in a closet that was observed to be unlocked.
Plan of Correction: A staff meeting was held to make sure to communicate if the med box is broken. A new med box has been provided at Widewater site and discussed the importance of keeping the med box locked.
