Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 12/03/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 13 children present, ranging in ages from 5 years to 12 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews it was determined that there were not always two staff members present when one or more children were in care. Evidence: Staff A was observed caring for 13 children alone during the morning operating hours of the school age program on Friday, December 3, 2021. Staff reported that the second staff person scheduled to work did not report for their shift.
Plan of Correction: Administration is in the process of hiring additional staff to have substitutes on hand when staff are unable to report to work.
