Inspector Notes

An unannounced monitoring inspection was conducted from 3:00 pm - 4:00 pm. There were 29 children in care being supervised by 2 staff. The children were observed participating in table-top activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-160-C
Based on review of three staff records it was determined that not all records contained documentation of a TB (tuberculosis) screenings every two years. Evidence: The most recent TB screening on file for staff A is dated 12/23/2020.
Plan of Correction: Staff A will obtain a new TB screening.
Standard 8VAC20-780-510-P
Based on review of medication it was determined that not all medication was picked up with 14 days of expiration of the authorization for medication form. Evidence: The authorization form for child A expired 09/05/2021. The authorization form for child B expired 01/21/2023.
Plan of Correction: Administration will request that the parents of child A and child B complete a new authorization form for their medications.
Standard 8VAC20-780-550-F
Based on review of documentation it was determined that lockdown procedures were not practiced annually. Evidence: There was no documentation on the drill logs for the year 2022 and 2023 that indicated a lockdown drill had been completed.
Plan of Correction: The facility will conduct a lockdown drill.
