Inspector Notes

An unannounced monitoring inspection was conducted from 4:00 ? 4:45 pm with staff. There were 22 children, ranging from six years to 11 years of age, present with three staff supervising. Children were observed participating in group exercises and playing with age-appropriate toys in a variety of centers. Three onsite staff records and two onsite child records were reviewed at the time of the inspection along with the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary restrictions, the first aid kit, emergency supplies, one medication and the required posted information. Four staff records were reviewed off site at the Massad YMCA on 2/7/2023. Based on review of the written attendance record, and an interview with staff, it was determined that the facility did not maintain a written record of daily attendance that documents the arrival and departure of children in care as it occurs. Evidence: 22 children were observed, however only 13 children were included on the attendance sheet.
Plan of Correction: Retrained staff to ensure that all children are signed in and out as they arrive and depart.
