Inspector Notes

This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 05/18/2021 and was conducted on 05/21/2021. The provider was contacted by telephone to initiate the inspection. There were no children in care and 1 provider present during the inspection. The inspector reviewed 2 children's records, 1 provider record, 1 pet record, and 1 household member record during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review and discussion, the provider failed to maintain a current certification in cardiopulmonary resuscitation (CPR).

Evidence: The most recent CPR certification obtained my Caregiver #1 expired on 01/01/2020.

Based on record review and discussion, the provider failed to maintain a current certification in first aid.

Standard 22VAC40-111-180-A
Based on record review and discussion, the provider failed to ensure that each caregiver and adult household member obtains a tuberculosis screening every two years from the date of the first screening.

Evidence:
1. The record for Caregiver #1 contained documentation of a tuberculosis test dated 06/13/2018; there was not a follow-up screening for 2020.
Plan of Correction: Will discuss getting a TB test with her doctor.
Standard 63.2-1721.1-A
Based on record review and discussion, the provider failed to ensure that all family day home providers shall undergo a background check in accordance with the Code of Virginia prior to employment and every five years thereafter.

Evidence: The record for Household Member #1 contained findings from a search of the central registry dated 02/10/2016; there were not follow up search of the central registry findings for 2021.
Plan of Correction: Will send off for one.
