Inspector Notes

A monitoring inspection was conducted on 5/26/2023 from 12:45 pm to 1:50 pm. There were no children in care. The Licensing Inspector reviewed three children?s records, one provider record, one household member record, and other program records.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on document review, the provider failed to ensure that subsequent background checks were done every five years for the provider and a household member over the age of 18.

EVIDENCE:
1. The most recent criminal history eligibility letter for Staff 1 was dated 3/9/2018. The most recent Central Registry ?No Match? letter was dated 1/10/2018.

2. The most recent criminal history eligibility letter for Household Member 1 was dated 2/16/2018.
Plan of Correction: The provider will request and obtain the required background checks.
Standard 8VAC20-800-180-A
Based on record review, the provider failed to ensure that a household member had a current, subsequent TB screening. These are due every two years.

EVIDENCE:
The most recent TB screening for Household Member 1 was dated 6/15/2018.
Plan of Correction: The provider will have the household member obtain a TB screening.
