A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-560-A
Each staff member shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment.

The screening for Staff #1 was completed two months after employment; Staff #2, who had been employment for seven days, did not have a completed screening on file; and the screening for Staff #3 was completed five weeks prior to employment.
Plan of Correction: In the future, staff will provide a completed screening within the required timeframe.
Standard 8VAC20-790-600-B
Staff are to complete the Virginia Preservice Training for Child Care Staff within 90 days of employment.

Staff #1, who had been employed for five months, had not completed the training, and Staff #4, who had been employed since 2008, did not have documentation that the staff completed the training.
Plan of Correction: In the future, staff will be required to complete the required training within 90 days of employment.
Standard 8VAC20-790-600-C
Staff is required to complete orientation training for staff prior to the staff member working alone with children and within seven days of the date of employment.

Staff #1, who had been employed for five months, did not have documentation of completing orientation training.

Staff #3, who had been employed for five months, did not have documentation of completing orientation training.
Plan of Correction: In the future, the center will ensure staff complete the orientation documentation form to ensure the required topics are covered and in a timely manner.

The center is required to maintain a record of the dates of the practice drills for one year.

The center did not have documentation that the shelter-in-place procedures were practiced twice per year in 2023.
