Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/03/2025. A POC submitted after this date will not appear on the public website.

Subsequent TB screenings are required at least every two years from the date of the initial screening, or more frequently if recommended by a physician.

Staff # 2?s updated TB screening was due three months ago. The updated screening for staff # 2 was not completed at the time of inspection. Staff #5?s updated TB screening was due eight months ago. The updated screening for staff #5 was not completed at the time of inspection.

Staff who work directly with children shall annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course.

Staff #1, employed for one year and seven months, did not have completed annual health and safety update course for 2024.
Staff #2, employed for over two years, did not have completed annual health and safety update course for 2024.
Staff #4 last completed the department?s annual health and safety update course on 05/11/2023.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

In the restroom used by children, a can of air freshener was located on top of the toilet that was not locked and was accessible to children.
Plan of Correction: Per the administrator: Air freshener was removed immediately. We will make sure all air fresheners are locked away.
