Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on May 10, 2021 and concluded on May 13, 2021. The director/owner was contacted by telephone to initiate the inspection. There were 36 children present and 8 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on review of children's record and interview, the center did not ensure to obtain all required information.

Evidence: In Child#3's record (start date:4/26/21), the name, address, number of another contact person to call in an emergency if a parent cannot be reached was not available for review. Interview with staff confirm this information was not available during inspection.
Plan of Correction: Will obtain the information for an emergency contact today. ( will contact parent)
