Inspector Notes

A monitoring inspection was initiated on Tuesday, August 2, 2022 and concluded on Monday, August 8, 2022. There were 76 children present with 11 staff supervising including administrative staff onsite. The
inspector reviewed compliance in the areas of administration, physical plant, staffing and
supervision, programming, special care and emergencies and nutrition. A total of
4 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-440-B
Based on observation and interview, the center did not ensure all cribs are identified for use by a specific child.

Evidence: In the infant room, Child#2 was observed in a crib that was not identified for use by a specific child. Interview with staff confirmed the use of the crib by Child#2 and not identifying it for child's specific use.
Plan of Correction: Retrain staff on proper sign in and sign out procedures in the classrooms.
Standard 8VAC20-780-500-A
Based on observation and interview, the center did not ensure children's hands shall be washed with soap and running water after toileting.

Evidence: In the Infant Room, Staff#1 observation of diapering was completed and Child#1 hands were not washed after diaper change. Interview with staff confirmed Child#1's hands were not washed after diapering

Based on review of documentation and interview, the center did not ensure to maintain a written record that documents the arrival and departure of each child in care as it occurs.

Evidence:
In Toddler's classroom, there were six children observed in the classroom. The attendance log showed eight children in the group. Interview with staff confirmed the count of six present in care and written attendance log not being update to reflect all of the children's for each group.
