Inspector Notes

A complaint inspection was initiated on August 2, 2022 regarding concerns of staffing and
supervision, physical plant and programs. Interviews with staff and collateral were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
