Inspector Notes

An unannounced non-mandated complaint montiroing inspection was initiated on April 30, 2024 from 9:45am- 11:21am. The complaint was in regards to administration, supervison, programs and special care provision and emergency. Staff were interviewed and records were reviewed. The inspection was concluded on 5/17/2024. There was preponderance of evidence in regards to administration and programs which determines complaint is valid in those areas. There was not a prepoderance of evidence with supervision and special care provision and emergency.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff interview and record review, the licensee did not ensure parents will be provided at least semiannually in writing information on their child's development, behavior, adjustment, and needs.

Evidence: Staff #1 confirmed , the center did not provide semi annually in writing information on children development, behavior, adjustments and needs.
Plan of Correction: An Assessment form is being adapted to be used semiannually.

Based on staff interview and record review, the licensee did not ensure children's records shall be made available to parents on request.

Evidence : During the inspection on April 30, 2024, Staff #1 confirmed the record was not provided to parent .
Plan of Correction: Records were retrieved from storage, scanned and emailed to parent. The child was disenrolled in August 20, 2023.
