Inspector Notes

An unannounced complaint inspection was conducted on 06/13/2024 in response to a complaint received regarding administration, programs, physical plant, staffing and supervision and special care provision and emergencies.

Based on the information gathered which included staff interviews and observaton this complaint is found to be valid in regards to administration.

Violation was found in the area of administration . The violation is listed on the violation notice and issued to the facility.

Based on documentation review and staff interview, the licensee did not ensure center informed the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Evidence:
An injury occured on June 3, 2024 while the child was under supervision of the center. The center called 911 and child went to the hospital. Person in charge went with the child in the ambulance.

The center did not report the injury to licensing inspector/department representive with in two buisness day.
Plan of Correction: The center will inform department a soon as possible but not exceed 2 buisness days for injury that occur and recieve outside medical treatment if needed.

The center has been provided guidance on how to report children injuries that need outside medical treatment on the ChildCare VA website.

The center immediatley purchased finger pinch guards for the doors after the incident happened.
