An unannounced inspection was initiated on 02/18/2025 and completed on 3/25/2025 in response to a complaint received by the licensing office on 02/07/2025 relating to allegations of staffing and supervision, special care provision and emergencies and programs. The inspector reviewed 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations .

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
