Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and 03/31/2025 completed on 03/31/2025. The on-site inspection began at 9:46am and ended at 12:16pm. The inspector reviewed
compliance in the areas listed above. There were 30 children present and 9 staff. The inspector
reviewed 7 children?s records and 5 staff records on-site. This inspection included document review , tour of the facility, interviews, observations , and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 16, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

In classroom 4, there was air freshener on top of the cabinet.
In classroom 5, there was a cabinet unlocked with hazardous substances.
Plan of Correction: Air fresher was immediately removed . Staff was reminded to always lock cabinet back after removing items from cabinet.

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

The classroom 5 had a two outlets uncovered.
The classroom 4 had one outlet uncovered.
Plan of Correction: All electrical outlets were recovered during inspection . Staff will double check daily to ensure outlets are covered.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Child #7 medication authorization expired 10 weeks ago. Medication was still at the center and not disposed of by the center.

Child #8 medication authorization expired 6 weeks ago. Medication was still at the center and not disposed of by the center.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

In the classrooms 4, there were nine children in the classroom and ten children checked in. Child #1 was moved to classroom 3.
Plan of Correction: Staff were reminded to sign in/out when children are moved to another classroom.
