An unannounced inspection was initiated on 09/26/2025and completed on 10/07//2025, in response to a complaint received by the licensing office on 09/17/2025 relating to allegations of program, special care provision and emergencies, and administration.This inspection included document review, tour of the facility, interview,s and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the
inspection determined no violations with applicable standards or law. No violations were issued.
