Inspector Notes

Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on September 19, 2022 from 11:15 a.m. through 12:55 p.m. Upon arrival, there were 32 students ages 2 through 5, in the care of 7 staff. Three children and three staff records were reviewed and updated. The medication and medication paperwork was reviewed for three children.
Areas observed were: educational centers, snack, hand washing/bathroom procedures, story time, transition, outside play and lunch. The center is well equipped with a variety of age/stage materials for the children in care.

Based on record review, the center accepted a tuberculosis screening for 1 staff that had not been completed within the last 30 calendar days of the date of employment.

Evidence:

1. Three staff records were reviewed.
2. Staff #2 had documentation of a tuberculosis screening dated 5/31/2022. Date of hire was documented on 09/01/2022.
3. The director confirmed the staff had not obtained a tuberculosis screening within the last 30 days prior to the date of hire.
