Inspector Notes

An unannounced monitoring inspection was conducted on September 20, 2023, from 11:00 a.m. through 2:00 p.m. Upon arrival, there were 50 children, ages 2 through 5, in the care of 8 staff.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 11 staff records were reviewed. One agent?s background information was also reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review the facility failed to obtain a repeated fingerprint check for one staff within 5 years of the prior one.

Evidence:

1. Review of staff records revealed Staff #2 had not obtained updated fingerprints within 5 years.
2. Staff # 2 's documented date of obtaining fingerprints was 07/03/2023.
3. The director confirmed Staff #2 had not yet obtained updated fingerprint checks
Plan of Correction: An appointment to obtain a repeated fingerprint check has been initiated. Documentation will be sent to the licensing inspector once received.
Standard 8VAC20-770-60-C-2
Based on observation the facility failed to obtain Virginia Central registry findings within 30 days of employment.

Evidence:
1. Staff record reviewed revealed Staff #6 and Staff #8 did not have documentation of obtaining Virginia Central registry checks within 30 days of employment.
2. Staff #6's date of hire was documented 03/01/2023. Staff #8's date of hire was documented 04/01/2023.
3. Director confirmed these central registry findings were not obtained with 30 days of employment.
Plan of Correction: Request for Virginia Central Registry findings has been mailed. Documentation will be sent to the licensing inspector once received.
