Inspector Notes

A monitoring inspection was conducted on 11/16/2022. The transition of part time children and stay and play children was observed. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Standard 8VAC20-780-160-A
Based upon review of staff records and staff interview, the facility has not ensured that the tuberculosis screenings for new staff are obtained within 30 days prior to employ.
Evidence:
1. The tuberculosis screening in the record provided for staff 3, employed on 8/1/2022, is dated 5/2/2022.
2. Staff 1 acknowledged that the tuberculosis screening on file for staff 3 was not obtained within 30 days prior to hire.
Plan of Correction: The program director responded with the following during the exit interview:
In the future, new employees will be required to obtain a tuberculosis screening within 30 days prior to employ.

Based upon observation and staff interview, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by the children.
Evidence:
1. Children were observed in a dance class in the gymnasium. The door to the electrical closet adjoining the gym was unlocked with sanitizing solution in the closet. The laundry room also adjoining the gym was unlocked with laundry detergent and other cleaning products in the room.
2. Staff 1 observed that the closets were not locked while the children were in the gym.
Plan of Correction: The program director responded with the following during the exit interview:
The closets were locked during the inspection. The custodial staff was reminded that all closets must be kept locked while children are in care. Staff have been reminded to check these closets to ensure they are locked prior to use of the gym by the children.

Based upon observation and staff interview, the facility has not ensured that a menu of snacks being served was dated and posted.
Evidence:
1. The menu posted outside the door to the Sandpipers classroom had only Monday's snack items indicated on the menu.

Plan of Correction: The program director responded with the following during the exit interview:
We will meet with staff to review the menu expectations and develop a plan to ensure that posted menus meet USDA requirements and are completed and dated as required.
