During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 3/13/2025 and completed on 3/13/2025 in response to a complaint received by the licensing office on 3/6/2025 relating to allegations of supervision of children. The inspector reviewed one children?s record and two staff records on-site on 3/13/2025. This inspection included document review, tour of the facility, interviews and observations.
The complaint investigation was reopened on 3/28/2025 and investigated with two outside agencies on 4/1/2025. During the investigation interviews were conducted, and video footage was reviewed. No additional violations were noted during the investigation, but the risk assigned was increased. The inspection was amended to include additional evidence found during the investigation to support the violation.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/7/2025. A POC submitted after this date will not appear on the public website.
Violations
1Child #1, 2 years old, ran through two doors located inside of the building, which led him outside of the facility. In video footage reviewed, you can see Child #1 run out the door and up the sidewalk, before getting into the main road with oncoming traffic in both directions. Staff can be seen coming out of the door, chasing the child up the sidewalk, and retrieving him from the road.
