Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.
A renewal inspection was initiated and concluded on 08/23/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 84 children present, ranging in ages from 4 months to 11 years, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, background checks and nutrition. A total of 4 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
1Evidence:
1. The record of staff #1 (DOH: 06/28/2021) contained a sworn statement that indicated that the employee resided in two states outside of Virginia within the past 5 years. For state #1, the criminal history results were received 7/7/2021. For state #2, the results of a criminal history record were not obtained.
2. For state #1, the results of a search of the child abuse and neglect registry or equivalent registry were missing.
3. Administration acknowledged that the results of the background checks were not obtained within the required time frames.
