Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated and concluded on 08/23/2021. There were 84 children present with 14 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the vendor did not ensure that each staff complete annually the Virginia Annual Health and Safety Update Course sponsored by the Department of Social Services as required.

Evidence:
1. The record of staff #3 (DOH: 09/05/2006) contained documentation of the Virginia Annual Health and Safety Update dated 07/13/2019.
2. The record of staff #4 (DOH: 03/05/2012) contained documentation of the Virginia Annual Health and Safety Update dated 07/24/2019.
3. The vendor acknowledged that the training update was not completed annually.
Plan of Correction: Staff 3 and 4 took the training on 8/23/2021 and submitted proof to the licensing inspector.
