Inspector Notes

The inspection findings have been updated as a result of a first-step review completed on October 23, 2024.

An unannounced inspection was initiated on 09/09/2024 and completed on 09/24/2024 in response to a complaint received by the licensing office on 08/19/2024 and 08/22/2024 relating to allegations of ratios and supervision of children. The inspector reviewed one child?s record, three staff records on-site and one staff record electronically on 09/23/2024. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violation(s) that were not reported were found related to procedures for emergencies and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/03/2024. A POC submitted after this date will not appear on the public website.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and must include all required information.
Evidence:
1. The injury report for an injury that occurred on August 14, 2024 did not contain the time when the parent was notified.
Plan of Correction: All accident reports will be reviewed by office staff.
