Inspector Notes

An unannounced, non-mandated inspection was conducted on 09/09/2024 in response to a complaint received on 08/19/2024. The investigation was focused on compliance with the vendor agreement (8VAC20-790). Information gathered during the inspection determined areas of noncompliance with applicable standards or law. Violations were documented on the Violation Notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
10/03/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-570-2
The vendor must inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.

Evidence:
1. Staff #1 stated they were made aware of the 08/14/2024 injury, where treatment from a medical professional was done, the next day on 08/15/2024.
2. No documentation or information was received by the department regarding the medical attention taken.
3. Leadership confirmed there was no reporting done for the injury on 08/14/2024.
Plan of Correction: When a parent notifies center of outside treatment, administration will ask for proof from parent and report to licensing agent.
