Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring/renewal/complaint) inspection was initiated on 08/05/2021 and concluded on 08/06/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 16 children present with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of three staff records and interview, the center did not ensure one staff member resubmitted the results of a follow-up tuberculosis (TB) screening every two years from the date of the first initial screening or testing.

Evidence: 1) The most recent TB screening in the record for Staff #2, hired on 07/05/2019, was dated 06/09/2019. The TB expired on 06/09/2021. 2) During interview, a member of management confirmed this is the most recent TB screening on file for Staff #2. Staff should resubmit TB test results at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department.
Plan of Correction: The staff member will go as soon as possible and will provide a copy of the results to the center.
