Inspector Notes

An unannounced, on-site monitoring inspection was completed on 03/12/2025. The on-site inspection began at 10:38 a.m. and ended at 2:47 p.m. The inspector reviewed compliance in the areas listed above. There were 14 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the
violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/22/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received immunizations required before attending the center. The record for C1, enrolled less than one month did not contain any immunizations.

The center shall obtain documentation of additional immunizations once every six
months for children under the age of two years. C3, who had been attending for seven months did not have documentation of additional immunizations.

Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of
employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. TB documentation for S4 was dated 36 days prior to date of employment.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside,
are maintained in a clean, safe and operable condition.
Electrical power strip in the Green Room was missing outlet covers.
The concrete support for entrance gate to the playground was not covered with materials to protect children from injury.
The mulch retaining timbers in the playground were loose in several areas creating
a tripping hazard.

The center shall maintain a current written list of all children allergies, sensitivities, and
dietary restrictions that is dated and kept confidential. The child allergy list posted in the center was dated for 08/19/2024 and did not document an allergy for C1 enrolled for less than 30 days.

The center shall maintain written records of children?s injuries that contains all of the
required information. Injury reports dated 11/04/2024, 11/19/2024, 01/03/2025, 01/23/2025, 01/27/2025, and 01/30/2025 did not document the date and time of parental notification of the injury. Injury report dated 01/27/2025 did not document any future action to prevent recurrence of the injury.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for C1 enrolled for less than one month was missing the written agreements between the parent and the center as required by 8VAC20-780-90 and the child?s doctor?s phone number.
