Inspector Notes
An unannounced inspection was completed on 07/08/2025 in response to a complaint received by the licensing office on 07/02/2025 relating to allegations of staffing and supervision, procedures for emergencies, and nutrition and food services. The inspector reviewed 4 child records on-site. This inspection included document review, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation is documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/18/2025. A POC submitted after this date will not appear on the public website.

4The required ratio for children 3 years to school age eligible is one staff per 11 children due to the ratio waiver being granted. Licensing inspector observed on 07/08/2025 at approximately 11:30 a.m. one staff in the Green Room supervising 13 children (youngest 3 years/3months).

When centers choose to provide meals or snacks, children shall be allowed second helpings of food listed in the USDA's child and adult care meal patterns. Staff were interviewed regarding children complaining of being hungry and no ?seconds? being offered. Staff reported that ?no seconds are offered?, ?normally we do not give them seconds?, and ?seconds are offered if there are leftovers.?
