Inspector Notes

An unannounced Code Compliance inspection was conducted with the Director on 4/25/2023 from 9:30am through 10:55am. There were 37 children present supervised by five staff. The children were observed playing outside and participating in circle time. Five child records were reviewed for documentation of immunizations and verification of proof of age and identity. Five staff records were reviewed for background checks, annual health statements, and training. The program has multiple staff with current certification in CPR and First Aid and one staff with Medication Administration Training. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not obtain health statements for all staff signed by a physician or physician assistant indicating that the staff is free from any disability which would prevent them from caring for children under their supervision.
