Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 10/23/2024 and completed on 10/23/2024, as a part of the licensure period. The on-site inspection began at 2:00PM and ended at 2:44PM. The inspector reviewed compliance in the areas listed above. There were 13 children present and 2 staff. The inspector reviewed 5 children?s records on-site 10/23/2024 and 4 staff records 10/10/2024. This inspection included - document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
