An unannounced, on-site monitoring inspection was initiated on 5/7/2025 and completed on 5/13/2025, as a part of the licensure period. The on-site inspection began at 2:28PM and ended at 3:15PM. The inspector reviewed compliance in the areas listed above. There were 19 children present and 2 staff. The inspector reviewed 5 children?s records on site on 5/7/2025 and 2 staff records electronically 5/13/2025.

Hazardous substances must be in a locked place that prevents access by children. Two bottles of Spic and Span with warning labels, ?Keep out of reach of children,? were unlocked in the cafeteria, within reach of children.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. During the inspection, Child #5 was unsupervised for approximately 9 minutes.
