REPEAT VIOLATION:
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
Three children (6 years of age), were unsupervised, as they transitioned from the gym and cafeteria for approximately 2 minutes. Staff #1 and Staff #2 did not have sight and sound supervision of the children.

The required ratio for children (school age eligible up to 9 years of age) is 1 staff per 18 children.

Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. Staff shall wash their hands with soap and running water before preparing or serving food or beverages.
The children (aged 5 years to 11 years) did not wash their hands following snack. Staff #1 and Staff #2 did not wash their hands prior to or after snack.
