Inspector Notes

An unannounced, on-site monitoring inspection was completed on 09/10/2024, as a part of the licensure period. The on-site inspection began at 9:40 am and ended at 11:35 am. The inspector reviewed compliance in the areas listed above. There were 50 children present and 13 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included document review tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on observation, the center did not obtain a copy of the results of a sex offender registry check, or a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding 5 years.

Evidence:
1. Staff #4 did not have results from the out-of-state sex offender registry.
2. Staff #4 did not have results from the out-of-state child abuse and neglect registry.
Plan of Correction: New Sex Offender and Central Registry searches have been submitted.

Based on a review of medications, a written care plan for each child with a diagnosed food allergy, was not updated annually.

Evidence:
Child #5's allergy care plan expired on 09/06/2024. The emergency medication was observed on-site during the inspection.
