The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #2, who had been attending for five months, did not have documentation of an immunization record on file. Child #4, who had been attending for two months, did not have documentation of an immunization record on file.
Plan of Correction: The center will obtain immunization records for the children.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Peeling paint was found in the center in areas accessible by children. A fence post on the playground needed repair.
Plan of Correction: The center will make the needed repairs.
