Inspector Notes

An unannounced, on-site renewal inspection was conducted on 10/10/2024. The on-site inspection began at 10:00am and ended at 11:25am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 14. The inspector reviewed 3 children?s records and 3 caregiver/household member(s) records. This inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

The physical for Child #1 was obtained almost 2 months after the first day of attendance.
