Inspector Notes

An unannounced Code Compliance inspection was conducted with the Director on 12/13/2022 from 9:30am to 10:30am. There were 37 children present, ranging in age from 2.5 years to four years old, supervised by six staff. The children were observed engaging in center play, coloring, doing Christmas crafts, and riding cars and bikes in the indoor play area. Five child records were reviewed for documentation of immunizations. Six staff records were reviewed for background checks, annual health statements, and training. There is an adequate number of CPR/First Aid and MAT trained staff on site. Areas of non-compliance are identified on the Violation Notice.

Based on record review, the center did not request an out of state child abuse and neglect search by the end of the 30th day of employment for all staff who have lived outside Virginia within the last 5 years. In addition, the center also did not obtain the results of an out of state criminal history record check prior to the first day of employment for all staff who have lived outside Virginia within the last 5 years.

Evidence: Staff #4 (Date of Hire: 9/12/2022) has lived in Connecticut within the last 5 years and there was no documentation that a request of the Connecticut child abuse and neglect registry has been requested by the center. Staff #6 (Date of Hire: 8/23/2021) has lived in West Virginia within the last 5 years and there was no documentation that a request of the West Virginia child abuse and neglect registry has been requested by the center or that the West Virginia criminal history record check results were obtained.
