Inspector Notes

An unannounced code compliance inspection was initiated on 7/22/2022 and concluded on 7/22/2022 from 9:10 AM to 10:05 AM. There were 11 school age children present with two staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and procedures. A total of two children?s records and three staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to obtain repeat background checks every 5 years.

Evidence:

1. The record for staff 1 contained a central registry background check dated 2/28/17.
2. The director verified an updated central registry background check had not been completed.
Plan of Correction: The director completed the request for an updated central registry background check on 7/28/22. In the future the director will ensure background checks are updated every five years for all staff.
