Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A monitoring inspection was initiated on 09/15/2021 and concluded on 09/16/2021. The director was contacted by telephone and a virtual inspection was conducted. There were thirty-four children present, ranging in ages from 2 years to 5 years, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of staff records, the center failed to ensure that an out of state criminal history name check required for non-NFF states prior to the 1st day of employment.

Evidence:
1. Staff #3's hire date was 08/09/2021. Staff #3 indicated on sworn disclosure statement dated 07/06/2021 that lived out the state of Virginia within the last five years.
2. The out of state criminal history name check was dated 06/12/20219.
3. The administrator confirmed that the out of state criminal history name check was completed by the old facility and not the current program.
Plan of Correction: The administrator sent out a new out of state criminial history check immediately. I will make sure that when it is required we will get our own checks and not depend on the ones from other facilities.
