Inspector Notes
A non-mandated inspection was conducted on June 10, 2024 from 11:25 A.M.-11:50 A.M, and concluded on June 25, 2024. The investigation was in response to a self reported incident that occurred on May 23, 2024 regarding supervision. The inspector reviewed compliance in the , staffing and supervision. Staff were interviewed and one childsrecords were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today . It will be due July 10, 2024. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
1Based on interview, the center failed to always ensure sight and sound supervision for children under the age of 10.
Evidence:
1. On 05/23/2024, Staff 2 opened the locked door to allow entrance to Child 1 (age 3 years) who was being dropped off by the parent. Staff 2 left Child 1 unsupervised at that time and child 1 walked up the stairwell. Child 1 was standing alone at the top of the stairwell for under a minute before the child was seen by Staff 3.
