Inspector Notes

This renewal inspection was initiated by licensing staff and concluded on 03/03/2023, between the hours of 9:30 am to 12 pm. There were 9 children present (23 points), ranging from infant to preschool-age, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 9 children records, and 3 staff records were reviewed.

Information gathered during the inspection determined that there were (4) non-compliances with the applicable standards, code, and/or law. These violations were documented on the Violation Notice issued to the home.

Standard 22.1-289.035-B-2
Based on review, 1 of 3 staff files indicated that a fingerprint check was unavailable prior to reporting to work on-site at the Family Day Home (FDH).

Evidence -
1. On the date of inspection (03/03/2023) a fingerprint check for Staff #3 was unavailable for review.
2. The start date for Staff #3 was listed as 03/03/2023.
Plan of Correction: A FPC for Staff #3 will be conducted and placed on file. Staff #3 was brought on in an emergency situation.

Based on review, 1 of 3 staff files indicated that not all of the required information was contained within each file.

Evidence -
1. On the date of inspection (03/03/2023), the following information was unavailable for review within the file for Staff #3: proof of age, 2 references, and emergency contact.
2. The start date for Staff #3 was listed as 03/03/2023.
Plan of Correction: Information for Staff #3 will be documented within their file.
Standard 8VAC20-800-210-A
Based on review, 1 of 3 staff files indicated that a caregiver did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence -
1. On the date of inspection (03/03/2023), there were no clock hours of annual training recorded in 2022 for Staff #2.
2. Certifications of training in 2022 were also unavailable for Staff #2.
Plan of Correction: It is unclear if Staff #2 will return. If they return, than 16 hours of annual training will be obtained for this year.
Standard 8VAC20-800-90-C
Based on review, 1 of 9 children's files indicated that the FDH did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) for children under 2 years of age, every 6 months.

Evidence -
1. On the date of inspection (03/03/2023), the immunization page on file for Child #4 was dated - 08/05/2022.
2. On the date of inspection, based on the information on file, Child #4 was under 2 years of age.
Plan of Correction: The parents of Child #4 will be notified and the updated immunization record obtained.
