Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 09/12/2024 and completed on 09/12/2024. The on-site inspection began at 10 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were (4) children present and (1) caregivers; the point total was (13). The inspector reviewed (5) children?s records and (1) staff records [on-site] on 09/12/2024. This inspection included:

? document review, policies and procedures, emergency drill logs, medication administration logs (no meds on site), authorization forms, 1 household record,
? tour of the facility, and
? interviews.

Information gathered during the inspection determined non-compliance with the applicable standards, and the violations were documented on the violation notice issued to the program.

Based on review, 1 of 5 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (09/12/2024), the policy number for the listed medical insurance policy name for Child #3 was not available for review.
2. The start date for Child #3 was listed as 08/05/2024.
Plan of Correction: Will get this missing information.
Standard 8VAC20-800-90-C
Based on review, 1 of 5 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (09/12/2024), an updated immunization page for Child #2 was not available for review (immunization page on file was dated - 10/25/2023).
2. On the date of inspection, based on information on file, Child #2 was under 2 years of age.
