During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/10/2025. The on-site inspection began at 9 am and ended at 11 am. The inspector reviewed compliance in the areas listed above. There were (5) children present and (1) caregivers with (1) volunteer; the point total was (13). The inspector reviewed (5) children?s records, (1) caregiver and (1) household member record on-site on 03/10/2025.

This inspection included: document review (i.e. files, Injury logs (no injuries), policies and procedures, emergency drill logs, no medication); tour of the facility; interviews; and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Documentation of additional immunizations is required once every six months for children under the age of two years. '

Child #1, 20 months old, last documented immunization was over 8 months ago.
Plan of Correction: Will obtain update from parent at the child's 2 year visit.
