Inspector Notes

An unannounced monitoring inspection was conducted on 02/10/2023 between the hours of approximately 11:55 a.m. and 12:45 p.m. There were 9 children present with the Provider and 1 assistant for a total of 20 points within the supervision guidelines. The children were observed playing outside. Positive interactions were observed during this inspection. A complete inspection of the physical plant, caregiver and children records, fire drill log, and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and provider interview, caregivers did not obtain a minimum of sixteen clock hours of training annually.

Evidence:
1. Caregiver #1?s record did not have documentation of training for the 2022 calendar year.
2. Caregiver #1 stated that they did not complete trainings in 2022.
3. Caregiver #2?s record did not have documentation of training for the 2022 calendar year.
4. Caregiver #1 stated the Caregiver #1 did not complete trainings in 2022.
Plan of Correction: We are already in the process of completing our trainings for this year.
Standard 8VAC20-800-60-A
Based on record review, the provider did not obtain all information for children?s records.

Evidence: Child #3?s record did not contain a name, address and telephone number for a second emergency contact.
Plan of Correction: I will ask for it.
