Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/11/25 and completed on 9/11/25, as a part of the licensure period. The on-site inspection began at 8:15am and ended at 9:10am. The inspector reviewed compliance in the areas listed above. There were 39 children present and five staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-60-A
Each center must maintain and keep at the center a separate record for each child enrolled that contains the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

The record for Child #2, enrolled for twenty-four days, was missing the name, address and phone number for 1 of 2 emergency contacts.
Plan of Correction: Will get the parent to complete the information for the 2nd emergency contact.
