Inspector Notes

A monitoring inspection was conducted on 07/11/2022. There were a total of 33 children with 6 staff members present. Children were observed making rockets and painting. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of medication, The center did not receive a label for the prescription medication that was on site.
Evidence:
Child #5 did not have a label on the prescription medication.
Plan of Correction: I will have mom bring it.

Based on file review, the center did not have documentation of health information as
required by 8VAC20-780-130 and there was no allergy care plan for a child with listed allergies.
Evidence:
1. Child #1 did not have a physical or immunizations on file during the inspection.
2. Child #2 did not have a physical or immunizations on file during the inspection.
3. Child #3 did not have an allergy care plan for a child with listed allergies.
