Inspector Notes

An unannounced monitoring inspection was initiated on 7/31/2025 and completed on 7/31/2025 in response to a self-report received by the licensing office on 7/25/25 relating to areas of the standards for physical plant, staffing and supervision, and special care provisions and emergencies. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/6/25. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center, inside and outside, must be maintained in a clean, safe and operable condition.

On 7/24/2025, child A sustained a cut to the palm of her hand while reaching for a Lego piece on the floor. A tiny shard of glass was embedded in the child's palm causing the injury. The fragment was a remnant from a glass mason jar that shattered during an art project the previous day.
