Inspector Notes

An unannounced monitoring inspection was conducted on May 1, 2020. There were 16 children present with 3 staff supervising. The physical plant, children's records, staff records, evacuation logs, attendance logs, menus, first aid kits and emergency preparedness information was reviewed. The children were observed handwashing, playing games and preparing to go to the gym. Areas of non-compliance were cited on the violation notice. Based on a review of children's records, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend.

Evidence:
Child #3's record does not have documentation of immunizations
Plan of Correction: I will obtain the immunization record for the child and place in the child's record.
Standard 8VAC20-780-140-A
Based on a review of children's records, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician.

Evidence:
Child #3's record does not contain documentation of a physical examination.
Plan of Correction: I will obtain the child's physical and place in the child's record.

Based on a review of staff records, the center failed to obtain all required staff information.

Evidence:
Staff #1, #2 and #3's records did not contain documentation of the address of a person to be notified in an emergency.
Plan of Correction: I will obtain the addresses of emergency contacts for all staff and place in the records.
