Inspector Notes

An unannounced monitoring inspection was initiated via email on 09/29/2025 and completed on 9/29/2025 in response to a injury of a child requiring outside medical treatment received by the licensing office on 9/26/2025 relating to Special Care Provisions and Emergencies. The inspector reviewed the incident form electronically on 9/29/2025. This inspection included document review.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.

The center shall notify the parent by the end of the day of any known minor injuries.
An injury report requiring outside medical treatment was received on 9/26/2025. The report stated that a child injured their arm on 9/23/2025 around 2:45 pm. The parent was not informed of the injury until 8:45 am on 9/24/2025. Staff did not inform the parent by the day of the day of the known injury.
Plan of Correction: We will use the SEMS system to locate the parents contact information and notify the parent by the end of the day. Also, we know the at the end of the day we will submit an injury report via e- incident report electronically.
