Inspector Notes

An unannounced monitoring inspection was conducted on May 3, 2023, from 10:34am to 11:44am with the provider and one assistant. There were eight children, ages 2, 2, 2, 3, 4, 4, 5, and 5 years old, were in the care of one caregiver for a total of 14 points. The children were observed coloring work sheets and playing with Play-doh. Four children?s records were reviewed. The provider record and two household records were reviewed and in compliance. One pet record was reviewed and in compliance. The provider has current certification in CPR and First Aid. The first aid kit, flashlight, and battery-operated radio were observed.

Standard 8VAC20-800-60-B
Based on a review of documentation, the provider did not ensure that all of the children's records contained the required information. Evidence: Child A and B's records were missing child's full name, nickname (if any), sex, address, and birth date; emergency contact information; first and last dates of attendance; proof of the child's age and identity. Child C's record was missing the insurance policy number and information for parents.
Plan of Correction: The provider stated that she would request the missing information from the parents and make sure it was placed in the children's records.
Standard 8VAC20-800-830-A
Based on a review of documentation, the provider did not ensure that an emergency evacuation practice drill was conducted on a monthly basis. Evidence: The provider did not have any documentation of monthly practice drills being conducted.
Plan of Correction: The provider stated that she would make sure to conducted and document emergency evacuation drills with the children on a monthly basis.
Standard 8VAC20-800-830-B
Based on a review of documentation, the provider did not ensure that shelter-in-place procedures were conducted twice a year. Evidence: The provider did not have documentation of a shelter-in-place drills being conducted.
Plan of Correction: The provider stated that she would conducted and document shelter-in-place drills twice a year.
